Health sector and municipality plan massive clean up effort in several port colonies

Mazatlan, Sinaloa. – Authorities of the Health Sector and the municipal government of Mazatlan remain on alert for a possible outbreak of dengue in the city, entering the months with the highest risk of the year and being very close to the epidemic zone in which Escuinapa is located and Rosario, with incidence rates of 33.48 and 7.57, respectively.

To date, Mazatlan is in second place in the table with more cases of dengue in the entity, adding 6 confirmed, out of a total of 628 probable; Escuinapa precedes it with 20, while Rosario occupies the third place with 4 cases, there are 9 more accounted for that are distributed in the rest of the municipalities of the state.

Since April no case of dengue was registered in the port; However, at the beginning of October, one more was confirmed in the downtown area, one of the most cared for in Mazatlan by its population, with more than 100,000 inhabitants.

As of this month, reports of suspicious cases rose from 2 or 3 that were given earlier, to 8 per week, today.

Alfonso Sandoval Sánchez, director of Municipal Medical Services, confirmed that he is about to initiate a program of a massive clean up in the main risk points of the city, to reduce the reproduction of the dengue transmitter fly, named Aedes aegypti.

After the rains, they are the periods with more risk, but we are about to initiate a massive event of scrapping by several parts of the city, we already have the sites located through the Ministry of Health and where they are more conflictive, and that we must attend with more haste.

For his part, Víctor Arredondo Rojas, responsible for the Department of Vectors of the Ministry of Health in the southern zone, reported that, although Mazatlan is still in the security zone, it is very close to Escuinapa and Rosario where the combination of the serotypes 1 and 2, with the most severe cases.

He said that the priority is the downtown area with its more than 100 thousand inhabitants, as well as the colonies that are on the banks of the Infiernillo estuary, including La Montuosa, Estero, Independencia, Libertad, Francisco Solís, Insurgentes, Toledo Corro , Juarez and Francisco I. Madero.

To strengthen the operational capacity and deal with a possible epidemic, on October 16, 42 new elements will be added to the brigades that will be in action until December 15, when the risks decrease when the cold enters.

LATENT RISK

Arredondo Rojas said that all preventive actions can be carried out by the authority in regards to scrapping, fumigation and dejection, but if the company does not participate in cleaning its yards, the risk will remain latent.

He informed that at week 40 of the current year, with the closing of October 10, the work of scrapping and elimination of hatcheries in the southern zone amounted to 1,064 tons, of which 558 were eliminated in Mazatlan, 361 in Escuinapa, 76 in Rosario , 58 in San Ignacio and 21 in Concordia.

In Mazatlan, he added, larval control has coverage of 147,600 homes worked in 32 colonies.

Every Friday there is an elimination of hatcheries in a different colony of the city, and between 12 and up to 30 tons are taken out, depending on the response of the citizenry.

He said that in the most recent case it was carried out in the Lázaro Cárdenas neighborhood, where more than 40 tons were taken out, with the participation of public works dump trucks and personnel from rehabilitation centers such as Uneme, CAPA and CEPTCA.

The strategy is to work in the center of the city and adjacent colonies to the estuary of the Infiernillo, as well as points where there is a lot of visitors.

“By working these colonies we protect the entire city, since most of the active population converges towards the center, and, therefore, if the Center of the city is uncontrolled, the cases are branched to the periphery, we have been avoiding the circulation of other serotypes that exist in other states, and that this comes to cause an epidemic ”.

He said that the highest risk is that serotype 2 arrives in Mazatlan, which is already in the municipality of Escuinapa, since the combination of 1 and 2 can cause a severe picture in patients.

“Where there is a combination of several serotypes is where the most severe cases of dengue are occurring, in fact, where serotype 2 is, it is where there are more cases like in Escuinapa, where up to 20 are confirmed,” he said.

Víctor Arredondo said that, in the state of Jalisco, where there are 3 serotypes (1, 2 and 3), in a single week a thousand confirmed cases were reported.

Fortunately, he added that, in the southern zone, and especially in Mazatlan, there is not so much resistance for the brigades to visit the homes, since out of every 100 houses visited, 70 or 75, the rest of the homes, most are alone.

Not so in other municipalities such as Culiacán, where brigades only have access to 50% of the homes they visit.

We have 13 reluctant for every thousand houses. Citizen participation represents 50% of dengue control, so we are confident that people here do participate, are in good communication with us, there are even people who call us directly and report cases of dengue suspects.

SURVEILLANCE BY AREA

He said that in addition to epidemiological surveillance, there is entomological surveillance, with 1,916 ovitrampas distributed in the city, which allow to know the population of mosquitoes that are in each area, taking into account the eggs that the female puts in the boats.

“A female has the capacity to lay 80 to 100 eggs every 3 days; the problem is that it does not put it in one place, the female disseminates it due to survival issues in batteries, tires, between 3 or 4 objects ”.

Up to 10 larvae can be developed in a soft drink bottle, while an average of 400 to 800 eggs have been found in a liter jar.

“We estimate that a car tire in the yard should have between 5,000 and 8,000 eggs, waiting for rainwater to fall to emerge mosquitoes.”

He said that the deposited egg can withstand up to one year drying; that is, if the female puts the egg in a tire that has no water in January or February, and does not rain until August or September, when the water falls, those eggs are viable for production.

Hence the importance of having clean the yards of pots, especially those that have a wall, which is where the females lay their eggs.

SEROTYPE 2, THE MOST DANGEROUS

Since 2015, Mazatlan has no outbreak of epidemic, even its highest incidence rate was in 2016 with 4.55, in that year 199 cases of dengue were registered here, of which 177 were non-serious and 22 patients presented symptoms of severe alert and dengue

We worry about the actions being done, but, even if we do the actions, there may be an outbreak, what we would like to avoid is the circulation of more serotypes. We have been lucky here in Mazatlan that we do not have a different serotype, because the entire population already had serotype 1, and if right now serotype 1 is circulating, there is not so much problem because most were already exposed to that serotype, but if comes 2, 3 or 4, practically whoever was bitten by a fly with another serotype is going to get dengue, it would be severe dengue by a combination.

He commented that serotype 3 has the capacity to be more virulent, but the most aggressive is 2, since it produces the severe conditions of the disease and to which dengue deaths are attributed, while 1 and 4 are the most noble.

Alfonso Sandoval said for his part that the population must be aware that the disease is in the houses, and that if you want to avoid an outbreak you have to clean the courtyards.

“The disease is at home, if they want to avoid it they can do it, how? You have to have a clean yard, free of pots where rainwater accumulates, which, if they have such pots empty and turn them over, the tires also accumulate a lot there, you don’t have to have tires that are useless, I I think we should start there, with the clean yards in our homes, ”he concluded.
